Invasive ductal carcinoma, CHG 1 of 3, with low grade DCIS.
Combined histologic grade is based on tubule formation (> 75% = 1, 10-75% = 2, < 10% = 3), nuclear pleomorphism (small uniform = 1, variable = 2, marked pleomorphism = 3), and mitotic count (0-1 = 1, 6-10 = 2, > 11 = 3) (the diamter of the objective has to be considered when doing mitotic counts). The addition of the individual scores is used in the combined histologic grade with well differentiated 3-5, moderate differentiated 6-7, and poorly differentiated 8-9 points.
