Outstanding Alumni Nomination Form

DO YOU KNOW SOMEONE WHO DESERVES RECOGNITION?

The School of Health Technology and Management Alumni Development Committee is now accepting nominations for the Outstanding Alumni Awards.

Nominee Eligibility Criteria:

  1. graduate of at least one SHTM degree or full time certificate program
  2. full time employment should NOT be within the SHTM (adjunct or guest faculty are eligible)
  3. demonstrate notable achievement in one or more of the following:
    • Leadership
    • Clinical Practice
    • Scholarship
    • Community service
    • Professional Service
    • School or University Service

We know that there are many exceptional graduates who deserve recognition. Please take the time to let us know about someone YOU know. Please complete the information below:
