Admission Application Instructions


Please PRINT and READ these two important documents BEFORE you begin applying on line. These step-by-step instructions will guide you through the application.

Rolling Admissions:  We will review your application when it is complete

Applications are available online. You may obtain applications by going to Office of Student Service’s web site:

ALL applications must be completed and submitted on line by the deadline. After that day, you will be “locked” out of the application. Late applications will not be considered until the next admissions cycle.

While on the application web site, remember to download three reference forms; these need to be completed if you are applying to the master's program. Completed, sealed reference reports should be submitted to the Office of Student Services (see address below). If a completed reference is given to you directly, DO NOT OPEN IT. Remember to sign the waiver section BEFORE you give a hard copy of the reference to be completed. If you are sending an electronic copy for someone to fill out, make sure you complete all sections.

GRE’s are required for the Master’s program. We cannot review your application until we receive official GRE scores. (Use institution code 2548 for Stony Brook when taking your GRE’s.) GRE’s are accepted if they have been taken within the past five years. GRE’s may be waived if the applicant has completed an accredited American graduate program prior to applying. Applicants who receive a 3.0 or less on the writing section of the GREs will be required to take a writing diagnostic course during the semester of the year they are accepted. Completion of diagnostic coursework is required prior to advancing to additional courses.

If you are currently a non-matriculated student, you are permitted to take a maximum of 12 credits total as a non matric. Failure to apply to the Advanced Certificate or the Master’s program will delay the completion of your degree. Upon completing 12 credits, you will be blocked from taking additional classes if you did not apply to the Advanced Certificate or Master’s. Complete and submit an Advanced Certificate or Master’s application to become matriculated.

For a student who is matriculated in the Advanced Certificate or who has completed the Advanced Certificate, no more than 18 credits may be applied to the Master of Science in Health Care Policy and Management.

You will need original official transcripts from ANY and EVERY college you have ever attended (yes, even if it was one summer course you took at ABC University – we need that official transcript). Stony Brook transcripts can be downloaded by Fran Shaw or Regina Biasetti – you do not have to request a Stony Brook transcript. If you transferred to Stony Brook, you must request official transcripts from your previous school(s). You should start requesting transcripts as soon as possible. Have transcripts sent to:
