Applications will be accepted for the fall, spring and summer terms with the following application deadlines.

Application supporting documents include:
- Completion of three question essay on online application
- Three (3) reference forms completed and submitted online (Not accepted from current SBU t-DPT students)
- Official transcripts from all colleges and universities attended
- Official evaluation of foreign transcript (course by course evaluation (WES, FCCPT, ICD, etc.) required)
- Photocopy of current United States physical therapy license/registration
- Proof of citizenship status, if applicable
- Resume

Non-Matriculation
If you missed the deadline date, you can still register for and start courses as a non-matriculated student and then apply to the program for the next admissions cycle. Non-matriculated students are permitted to register for up to a total of 12 credits. These credits may be applied to the post-professional program once a student has been formally accepted into the program. Students who wish to register as non-matriculated must complete the non-matriculated application form as well as the non-matriculated student health form and meningitis form.
